NOMINATION INSTRUCTIONS:

1. Entries for all categories are due by 5 p.m. on Thursday, Jan. 20, 2011. All entries must be submitted via dmagazine.com.
2. Address each criteria in narrative form and in the order requested. Narrative should contain complete, clear, and concise answers.
3. Supporting documents will be accepted as supplements to the required narrative. No more than four (4) pages of supporting documents are allowed. You will have the opportunity to upload supporting documents.
4. Applications are the responsibility of the nominating organization; incomplete applications will not be considered. Before nominating individuals, please confirm they will accept an award if chosen and if they are available to attend the awards event in mid-April 2011.
5. Please provide supporting details. For example, avoid submitting general information, instead use numbers and statistics. Make sure the judges have enough details to make informed decisions—but don’t overwhelm them.
